Dr. Jay Hazen, DDS

Dr. Hazen is a 1991 graduate of the Marquette University Dental School. He has several hundred hours of continuing education credits in all phases of dentistry including intensive training and education in aesthetic and reconstructive dentistry and is a current member of the following, Graduate of LVI – Las Vegas Institute of Advanced Dental Studies, American Association of Cosmetic Dentistry, Academy of Computerized Dentistry, Academy of General Dentistry, American Dental Association, Wisconsin Dental Association. Dr. Hazen has been named one of the top cosmetic dentsits in Madison by his peers.
